The Gillette Children's Pediatric to Adult Health Care Learning Collaborative Summit is the culmination of an eight-month, virtual learning series that brought together a multidisciplinary group to learn about health care transition (HCT) and discuss real-time HCT challenges. This learning series focused on sharing HCT resources and ideas grounded in health equity and trauma-informed principles. The Transition Summit builds on this series by bringing together leading experts, stakeholders, healthcare professionals, and others to share best practices, exchange knowledge, and drive advancement in the field of HCT for youth with special health needs. The Transition Summit will offer participants valuable insights into enhancing their clinics, hospitals, programs, and overall state practices related to pediatric to adult HCT. These advancements are urgently needed to support the growing number of youths with special health needs aging into adulthood and to ensure safe HCT from pediatric to adult care systems.

Presentations will focus on:

  • Extending the principles of patient- and family-centered care, trauma-informed care, and health equity to HCT.
  • Introducing state-level initiatives to advance HCT.
  • Best practices from other health care systems on implementing measurable and sustainable HCT processes.
  • Identifying how your organization can move the dial on HCT.

Along with Got Transition, you will hear from experts at the MN Department of Health, Family Voices, and organizations that successfully and sustainably implemented HCT processes at their organizations. Dr. White and Ms. McManus, along with clinical experts from Gillette Childrens, and other esteemed guests, will provide expertise and guidance to foster engaging dialogue centered on current conversations in pediatric to adult HCT.

Cancellation/Refund Policy: Cancellations received by 04/11/24 will be eligible for a full refund. Cancellations received between 04/12/24 and 05/16/24 will be eligible for a 50% refund. Cancellations received after 05/16/24 will not be eligible for a refund. All refunds issued will be less any applicable Eventbrite registration fees and will be issued to the original payment method used to register for the summit.
